Background
To promote the concept of age-friendly home through the community service collaboration, this program has been sponsored by several business partners in providing simple home-modification services to needy elderly since 2017. With such sponsorship, our occupational therapists conduct home assessment and purchase appropriate aids for elderly.

Aims
- To promote the concept of AFH in the community
- To identify "high risk" elderly and provide home modification services
Service Target
- Low-income elderly which are owners of private buildings, with high risk of falling, and interested in improving their home safety
 Occupational Therapist provides the home assessment to the elderly. |
 Assistive devices are purchased to the elderly at high risk of falling. |
